Toxoplasmosis presenting as a swelling in the ingunal area

Inguinal US 결과 Rt inguinal area에 최대 1.9cm 크기의 multiple enlarged LN 확인이 되었으며 상당수는 eccentric cortical thickened & fatty hilum이 없는 suspicious finding 보였습니다. 가장 큰, 만져지는 LN에 대해 ultrasound guided core needle biopsy (18G) 시행하였으며, reactive hyperplasia of lymph node로 보고 되었습니다. Tissue confirm 위해 excisional biopsy of Rt inguinal LN 시행하였으며, toxoplasmosis의 가능성을 배제하지 않을 수 없는, granuloma가 형성된 reactive hyperplasia로 확인이 되어 인근 대학병원에 의뢰를 하였습니다. 대학병원 감염내과에서 serology와 PCR을 통해 toxoplasmosis lymphadenitis로 확진 후 약물 치료를 하였습니다.

Toxoplasmosis lymphadenopathy는 조직학적으로 florid reactive follicular hyperplasia – epitheloid histiocyte 혹은 microgranuloma, absence of giant cell and follicular hyperplasia로 진단에 도움을 줄 수 있습니다.
